Recycle an old disk drive, take out the main lens and insert it into the hole in the printed table, place your smartphone on top aligning the lens with the camera, screw the nut onto the screw, the nut has a bed with holes, using some transparent tape or glass color what you want to see over one of the holes so the light can pass underneath. IMPORTANT: print the nut with as little filler as possible and the screw with at least 50% filler, it is important to keep the base heavy.
